The Reality
If you've thought about therapy and stalled, you're not alone.
These are the situations we help Philadelphia customers solve every week.
- →You've tried looking for a therapist before and burned out trying to find one with availability who takes your insurance
- →Your last therapist felt like a generic script — empathetic but not actually moving you anywhere
- →Your schedule won't allow weekly daytime appointments and most therapists' "evening hours" are 5pm
- →You've been told you might benefit from EMDR or trauma-informed care and most local clinics don't offer it
- →You've moved between PA, NJ, and NY and your therapist couldn't follow you across state lines
Our Services
What we work on together.

Service · 01
Individual Therapy (Anxiety, Depression, Stress)
Weekly or biweekly individual therapy for adults. Cognitive-behavioral, mindfulness-based, psychodynamic, and integrative approaches matched to what actually fits the person — not a one-size-fits-all protocol. Goal-setting and progress reviews every 8 weeks.

Service · 02
Couples & Relationship Counseling
Gottman-method-informed couples work for relationships in conflict, considering separation, navigating life transitions together, or just trying to communicate better. Both partners join the telehealth call from wherever they are. Evening and weekend slots available.

Service · 03
Trauma & EMDR-Informed Care
EMDR (Eye Movement Desensitization and Reprocessing), Internal Family Systems (IFS), and trauma-focused CBT for adults working through past trauma, PTSD, or unresolved grief. Pacing matters here — we move at your speed, not on a protocol calendar.

Service · 04
Life Transitions & Career Counseling
Therapy and structured coaching for career pivots, divorce and post-divorce identity, empty-nest transitions, retirement, and major life decisions that don't fit a clinical-disorder frame but still deserve thoughtful work. Time-limited engagements (8–16 sessions) available.
The Difference
Why people stay with Threshold.
Multi-State Licensure
Dr. Brennan is licensed in Pennsylvania, New Jersey, Delaware, and New York. If you move within the four-state region, your therapy doesn't end. If you split time between two of those states, we can see you in either. State-line continuity is a real reason clients pick a multi-state practice.
Evening & Weekend Slots
About 60% of our weekly slots are evening (after 5pm) or Saturday. We deliberately built the practice for working professionals, parents, and graduate students whose 9–5 isn't open. Same therapist sees you whether you book weekday or weekend slots.
Most Major Insurance Accepted
In-network with Blue Cross Blue Shield, Aetna, Cigna, United Behavioral Health, and Optum across our four-state region. Sliding-scale and out-of-network superbills available for plans we don't credential with. We verify coverage before your first session so there are no surprises.
EMDR + Trauma-Informed
Trained in EMDR, IFS, and trauma-focused CBT — not standard at every practice. About 30% of our work is trauma-related, including complex trauma that doesn't always fit a textbook PTSD diagnosis. We don't rush trauma work; we don't avoid it either.
Common Questions
You probably want to know.
How much does it cost?+
Self-pay rate: $185 per 50-minute session. With in-network insurance, your cost is whatever your plan's mental-health copay is (typically $20–$50). Sliding scale available — we hold a portion of weekly slots at $95–$135 for clients who don't have coverage.
Will my insurance cover this?+
We're in-network with BCBS, Aetna, Cigna, United Behavioral Health, and Optum across PA, NJ, DE, and NY. We verify your coverage before your first session — including deductible status, copay, and number of authorized visits per year. Out-of-network superbills available for other carriers.
How does telehealth therapy actually work?+
HIPAA-compliant secure video session via SimplePractice or Doxy.me — same look as a Zoom call, end-to-end encrypted. Typical session is 50 minutes. We send you a calendar invite with a unique link 24 hours before each session. Mobile, tablet, or laptop all work.
What if I'm in crisis?+
We're a weekly outpatient practice — not crisis services. If you're in an acute mental health crisis (active suicidality, psychosis, severe self-harm), the right care is 988 (Suicide & Crisis Lifeline) or your nearest ER. We can support you before and after a crisis, but during one, you need higher acuity care.
Can I switch therapists if it's not a fit?+
Absolutely — fit matters more than any clinical credential. Free 15-minute consultation before your first session is exactly so you can hear how Dr. Brennan works and decide. If after 2–3 sessions it's not the right fit, we'll refer you to a colleague whose style matches better. We've never lost sleep over a referral out.
Get a free 15-minute consultation. No commitment.
Brief intro call by phone or video — what's going on, what you're hoping to work on, and whether we're the right fit. If yes, we schedule. If no, we'll refer you to someone better suited. Either way, you get a clear next step.
